2 No Doctor-Patient Relationship
Browsing this website, submitting a contact form, requesting a consultation, or otherwise communicating with Monti Institute of Neuro Aesthetics through this site does not, by itself, establish a doctor-patient relationship. That relationship is formed only after a formal intake process, an in-person or telehealth evaluation, and mutual agreement between you and one of our licensed providers.
Information you submit through our contact form or scheduling system prior to that formal intake is not treated as part of your medical record and should not include sensitive health details. See our Privacy Policy for how pre-intake web inquiries are handled.
3 Treatment Risks
All medical and aesthetic procedures — including neurological treatments, injectable and energy-based aesthetic treatments, and infusion therapies — carry inherent risks. These may include but are not limited to allergic reaction, infection, bruising, swelling, asymmetry, and, in rare cases, more serious complications.
A complete discussion of risks, benefits, and alternatives specific to your case takes place during your in-person consultation and is documented in your signed informed consent prior to any treatment. No summary on this website is a substitute for that conversation.
4 Results & Photography
Individual results may vary. Outcomes are influenced by each patient's unique physiology, medical history, treatment plan, and adherence to post-care guidance. Photographs, before-and-after comparisons, and testimonials shown on this website reflect the experience of specific patients under the care of Monti Institute providers and are not a guarantee, warranty, or prediction of results for any other individual.
All patient photographs displayed on this website are used with the explicit, written consent of the patients depicted, obtained through a signed model release and clinical photography consent form prior to publication. Patients retain the right to withdraw consent and request removal of their images at any time by contacting our office.
5 Emergency Notice
This website is not monitored for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use this website, our contact form, email, or text messaging to seek emergency care or to report an urgent post-treatment complication requiring immediate attention.

6 Text Messaging
Appointment reminders and related text messages are governed by our full SMS & Text Messaging Consent Disclosure, including consent scope, message frequency, opt-out instructions, and data handling commitments. See Privacy Policy, Section 6 for the complete disclosure.
As a general safety note: standard SMS text messaging is not a fully encrypted communication channel. Please do not send sensitive medical details, symptoms, or protected health information by text message. If you need to discuss clinical details, call our office or use our secure patient portal.
7 Good Faith Estimate Notice — Your Rights Under the No Surprises Act
Under Section 2799B-6 of the Public Health Service Act, health care providers and facilities are required to give patients who are uninsured or who are not using their health insurance an estimate of expected charges for non-emergency items and services, including related costs like medical tests, prescription drugs, equipment, and hospital fees.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, in writing, upon scheduling care at Monti Institute of Neuro Aesthetics, or upon request.
- If you schedule an item or service at least 3 business days in advance, we will provide your Good Faith Estimate no later than 1 business day after scheduling. If you schedule at least 10 business days in advance, we will provide it no later than 3 business days after scheduling. You may also request a Good Faith Estimate before scheduling any item or service.
- The Good Faith Estimate will show the costs of items and services reasonably expected for your care, based on information known to us at the time of the estimate. It is not a bill, and actual charges for the care you receive may differ from this estimate if unforeseen circumstances arise during treatment.
- Keep a copy of your Good Faith Estimate. If the actual amount billed is $400 or more above your Good Faith Estimate, you have the right to dispute the bill.
How to Dispute a Bill
You may start a dispute resolution process by contacting the U.S. Department of Health and Human Services (HHS) at 1-800-985-3059. You must start the dispute process within 120 calendar days of the date on the original bill. There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on your Good Faith Estimate. If the agency disagrees and agrees with the health care provider, you will have to pay the higher amount.

8 Secure Portal & Web Contact Form — Scope of Use
Monti Institute of Neuro Aesthetics uses Boulevard for appointment scheduling and offers a secure patient portal for clinical communication. Both are encrypted, HIPAA-compliant channels intended for the exchange of appointment details and, where applicable, protected health information as part of an established provider-patient relationship.
The standard contact form on this website is not a secure clinical channel. It is provided for general inquiries, scheduling requests, and non-clinical questions only, and is transmitted over encrypted HTTPS but is not part of our HIPAA-covered systems in the way our scheduling platform and patient portal are.
Please do not submit sensitive medical information, symptoms, medication details, insurance information, or other Protected Health Information through the standard web contact form. Any such information volunteered outside of the designated secure patient portal or scheduling platform is submitted at the sender's own discretion, and Monti Institute of Neuro Aesthetics is not liable for the transmission or handling of sensitive clinical information sent through channels not designated for that purpose. If you need to share clinical details, please use our secure patient portal or call our office directly at (775) 000-0000.
9 Terms of Use — Governing Law & Intellectual Property
All content on this website — including text, graphics, photographs, the Monti Institute name and logo, and all associated brand assets — is the property of Monti Institute of Neuro Aesthetics or its licensors and is protected by United States copyright, trademark, and other intellectual property laws. No content from this website may be reproduced, distributed, modified, or used for commercial purposes without our prior written consent.
These Terms of Use, and any dispute arising from or relating to your use of this website, are governed by and construed in accordance with the laws of the State of Nevada, without regard to its conflict-of-law principles. Any legal action or proceeding arising from your use of this website shall be brought exclusively in the state or federal courts located in Washoe County, Nevada, and you consent to the personal jurisdiction of those courts.
10 Cancellation & No-Show Policy
We reserve dedicated appointment time exclusively for you. Late cancellations and missed appointments prevent other patients from being seen and affect the availability of our providers. The following policy applies to all appointments, including consultations, injectable and aesthetic treatments, infusion therapy, and neurological visits.
Notice Required
We require at least [24/48] hours' notice to cancel or reschedule an appointment. You may cancel or reschedule by calling our office at (775) 000-0000, through our online client portal, or by replying to your appointment confirmation text where enabled.
Late Cancellation & No-Show Fees
- Late cancellation (inside the [24/48]-hour window): a fee of [$X or X% of the scheduled service] may be charged to the card on file.
- No-show (missed appointment with no notice): a fee of [$X or X% of the scheduled service] may be charged to the card on file.
- Deposits collected at booking for certain aesthetic or infusion services are non-refundable if the appointment is cancelled inside the notice window, but may be applied toward a rescheduled visit at our discretion.
- Repeated late cancellations or no-shows may require full prepayment for future appointments, or, at the practice's discretion, result in dismissal from the practice for elective aesthetic services.
Exceptions
We understand emergencies happen. Fees may be waived at the practice's discretion for documented medical emergencies or other extenuating circumstances. Please contact our office as soon as possible if you are unable to give standard notice.
This policy is separate from, and does not affect, your rights under the Good Faith Estimate notice above — cancellation fees are not part of the estimated cost of a treatment and are billed independently as a scheduling fee.
